For some therapies, once a booking has been made, there is a mandatory health questionnaire that must be completed.
This is based on the fact that the client does not have any health issues or conditions that would make them vulnerable to further illness; if there are any discrepancies or possible issues identified in the form, then the therapy will not take place.
Failure to return the form prior to the therapy will result in a refusal of treatment - this is for 2 specific reasons:
- Preservation of your own health
- Insurance purposes
